Building & Grounds, Telephone Repair Request

If your telephone requires repairs or changes to its features, complete a Telephone Request Form.  For general inquiries, contact Raj Ramcharan or Jason Carmichel.

To access the Telephone Request Form, do the following:

1. Log into the Staff Intranet with your library barcode number and pin
2. Under Forms, select Telephone Request Form from the first dropdown menu internal equipment, facilities & services
3. Complete the form and submit
